Your First Visit

We welcome new patients, whether you need us for diagnosis, treatment of a chronic or acute condition, preventive care or human performance. Our treatment approach is customized based on your specific needs. Our goal is for every single patient that qualifies for our sample treatment session to experience some form of immediate improvement or relief. Improvement may come in the form of increased mobility, or a decrease in the quality or intensity of symptoms. We always reserve two appointment times per day to be sure we are able to accommodate those in urgent need of care and guarantee a new patient will be offered an appointment time within 3 business days. You can request an appointment by clicking here or by calling us at (408) 241-TEAM (8326).

When you walk into our office you will be greeted by our friendly staff. We encourage our new patients to arrive 15 minutes prior to the scheduled appointment time to complete initial patient forms that tell us about your current complaint and your past medical history. A thorough picture helps us to provide you with the best possible care. We can provide you with an examination gown, but we encourage new patients to wear comfortable athletic clothing that ensures easy access to all areas of complaint. We also have a dressing room available in case you are coming from or going directly to work. Please be sure to bring any past medical information, including imaging studies and reports, if available.

Next, you will be seen by one of our specialists. He or she will discuss with you the history of your complaint to understand your symptoms and how they are affecting your life. We will then perform the necessary orthopedic, neurologic and functional examinations to determine the cause of your condition and whether or not you qualify for care at our clinic. We may also take also digital x-rays if they are necessary.

If your history & examination findings qualify you for care, you will be directed to our therapy wing for your customized sample treatment which may include: joint mobilizations, massage, acupuncture, specific stretching and stabilization exercises, decompression therapy, electrical muscle stimulation, ultrasound, Graston Technique, Active Release Technique (ART), heat, ice, cold laser, and/or kinesiotaping. We have a wide variety of tools and modalities at our disposal in one facility and are able to provide a multi-faceted approach to your treatment.

The sample treatment serves two purposes: first, to give you an appreciation of our comprehensive services; and second, to confirm our initial clinical impression with a positive response to therapy. You can be assured that our sample treatments are safe, effective and comfortable.

Please plan on at least 60 minutes for your initial visit, possibly more, depending on the complexity and severity of your case.

Your Follow-Up Visit

The follow-up visit completes the new patient experience and is included as part of the initial visit fee. The doctor will sit down with you and present the findings of the physical exam, and assess your initial response to the sample treatment given during your first visit. The two of you will discuss your goals and expectations for treatment. With that information, the doctor will be able to design a tailored treatment plan outlining the frequency and duration of your care. If time allows and your elect to move forward with care, you may start your first full treatment plan session that same day. Your follow-up visit and subsequent treatment plan visits will last 35-60 minutes.

Treatment Plan

The wide array of conditions we treat require different levels of care, and on top of that, there is no one else out there that is just like you. This is why we always take a customized approach when it comes to designing your treatment plan. We will take into account your specific condition, level & type of activity, goals, schedule, and what types of treatment you benefit from the most to create a plan optimized just for you; no more than you need and certainly no less.

Home Care & Recommendations

What you do outside our clinic can be just as important as the care you receive inside. With that in mind, we always provide home care instruction and training recommendations for your condition as well as a game plan for a safe return to regular training and activities. This is included as part of your follow-up visit to our clinic to ensure you are recovering as quickly as possible and maximizing the benefit of your treatment.

Referral

Although most of our new patients come to us through the direct referral of friends and family of existing patients, you do not need a referral to be seen at our clinic.

We occasionally encounter a patient whose condition requires undergoing a procedure or care at a different facility. For these patients, we leverage our extensive referral network of trusted associates that we have developed over the last quarter century to make sure these patients receive the help that they need.

Financial / Payment Options

Payments

We require all our new patients to pay an initial fee of $195 unless you are admitted as part of a team promotion or with a gift card. If our services are covered under your insurance plan, the $195 will be applied to your plan deductible, co-payment, clinic treatment plan, or refunded to you after your insurance payment is received.

We do not want payment to stand in the way of you receiving care at T.E.A.M CLINIC®. If you have a special financial situation or don’t have health insurance, please talk with the office manager.
